Our referral form asks for:
This information is submitted by the referring dental office rather than by the patient.
Where it goes. Referral information is stored in the website's database. It is not sent by email. When a referral arrives, our office receives a notification containing no patient information, only a link to view the referral after signing in. This keeps patient information out of email inboxes.
Who can see it. Access is limited to practice staff who handle referrals and our website provider, and requires signing in.
How long we keep it. Referral submissions are automatically deleted from the website 14 days after they are received.
As with most websites, our host records standard technical information: IP address, browser type, pages visited, and the time of the visit. This is used to keep the site running and secure.
We use a consent banner so you can choose what is set. Necessary cookies keep the site working and remember your choice. Other cookies are set unless you opt out, which you can do at any time through the banner or the Cookie Preferences link at the bottom of every page.
Our homepage displays Google reviews through a third-party service, which may set its own cookies when that section of the page loads. Some pages include videos hosted on YouTube, and our location pages display Google Maps. Both may set their own cookies when they load.
We use Google Analytics to understand how the website is used, for example which pages are read most often and how visitors arrive. It collects information about website usage, not about patients, and you can turn it off by opting out of statistics cookies.

If you are a patient, your rights regarding your medical record are broader than this statement describes and are set out in our Notice of Privacy Practices.
This website is not directed at children, and we do not knowingly collect information from children through it. Referrals for patients who are minors are submitted by a dental office rather than by the child.
We use encrypted connections, keep patient information out of email, and require a password-protected login to view referrals, which is limited to our practice staff and our website provider. Referrals are removed from the website after 14 days. No system is perfectly secure, but we take reasonable measures to protect the information we hold.
